Getting Started

Scheduling a first appointment can be a difficult step to take in getting help. While many people put it off, it is better to get started and learn to manage the problems rather than allow them to escalate into larger problems. After the first appointment or at any point in the therapeutic process the patient is under no obligation to continue. Therapy is for the patient and it is important for them to feel as though they have made a good and careful choice. Questions are encouraged about the services and the therapist.

    NOTE: Do not use online scheduler if this is an emergency, use telephone contact and follow directives.

    Scheduler is extremely sensitive to various privacy and ethic standards for customer data storage and transmission. Data is deleted from storage upon retrieval.

    You will receive a password to use in downloading the intake packet when the appointment is confirmed through a confirmation telephone call.

Donwload Intake Packet

  1. Download the intake packet from the �Intake Packet� page using the password provided when scheduling.
  2. Complete an intake packet prior to the appointment for each member attending and bring it to the 1st appointment.
  3. If desired, download, print, and review the privacy practices from the Privacy Practices" page or ask for a complete set of Eastview Counseling Privacy Policies at your first appointment.
  4. Bring your insurance card if you plan to file insurance.
Directions

From north of Newton:
Take I-135 to Broadway exit. Turn left (east on the round-about) to Spencer Road (this will be a T intersection.). Turn left at the T and proceed until you cross a railroad track and stop sign. Drive across the paved road toward the north and turn right at the Eastview Counseling Sign. Proceed to the back entrance down the short hill.

From south of Newton:
Take I- 135 to Newton exit 31. Turn right (east on the round-about) to Spencer Road. Turn left on Spencer Road and drive 1.5 mile until you cross a railroad track and stop sign. Drive across the paved road toward the north and turn right at the Eastview Counseling sign. Proceed to the back entrance down the short hill.

Before your First Appointment

  1. Dr Koop will telephone you after you have scheduled online and provide a password with which you can download the intake packet.
  2. Download the intake packet from the �Intake Packet� page.
  3. Complete an intake packet prior to the appointment for each member attending and bring it to the 1st appointment.
